This page describes this website’s initial inquiry form. It is not a notice about clinical records or a substitute for the privacy information you receive when beginning therapy.

What the form collects

The form asks for your name, email address, a general reason for contacting Heather, and your permission to receive an email reply. Please do not put symptoms, diagnoses, treatment histories, insurance numbers, or other sensitive health information into the form.

How your inquiry is handled

Your inquiry is sent by email to the practice using Resend. Vercel hosts the website and processes the request. The application does not save form submissions to a database or browser storage, and does not intentionally include your submitted details in application logs. Hosting and email providers may retain operational records under their own policies.

Your contact details are used to respond to your inquiry. Email is not a confidential therapy service. Sending the form does not establish a therapist–client relationship or reserve an appointment.

Cookies and analytics

This application does not add advertising trackers, analytics tools, or its own cookies. Hosting and security providers may process technical information, such as network and request data, to operate and protect the service.

Questions or corrections

To ask about an inquiry you have sent, or to request a correction or deletion, reply to Heather’s response or use the contact form to request a conversation. Record retention and deletion may depend on applicable obligations and the systems that processed your inquiry.
